Risk and Exposure Assessment

Description

Used as part of the UAMS Occupational Exposure Assessment and Control Policy (Admin 11.4.04) to help identify and evaluate potentially hazardous workplace exposures to physical, chemical, biological and radiological agents. Department Managers are required under the policy to complete (or designate an employee to complete) this exposure assessment data collection form. One assessment per laboratory, unit, floor, etc. should be submitted to report the potential hazards and their risk levels to OH&S. The assessment should be completed by January 31st of each year.
