Radiation Dosimeter Request Form

Description

This simple web form generates an email to notify the UAMS radiation safety officers when an employee enters a request for a radiation dosimeter badge. A dosimeter badge tracks radiation exposure, which is used to calculate total exposure over time. Users enter basic information about themselves and fields about where they work and the Cost Center, Fund, and Subaccount to charge for their dosimeters. After they submit the request, they are directed to required training (if needed) in Workday and to go to TMA and enter a work order for their badge.
