The Formal Complaint Resolution internal Website has two distinct functions: An online form for UAMS students to enter formal complaints and a management tool for handling those complaints. On the form, students provide their contact information, describe the incident they're reporting, and can attach supporting documents. The administrative functions support the ability to route complaints to the appropriate department heads while time-stamping and capturing the response and adjudication for each complaint. The site generates an email to the Institutional Research, Policy, and Accreditation department within Academic Affairs when a complaint is entered. Users who enter their contact information receive a confirmation via email that their complaint was received.
