Description

Three versions of the Confidentiality Agreement web application are available for those who work for or do business with UAMS to obtain certain authorizations or permissions. A current Confidentiality Agreement is required as a condition to work for or do business with UAMS (for non-employees, such as vendors or contractors). The form describes a commitment to obey confidentiality laws and maintain the confidentiality of patients' Protected Health Information (PHI) in any form. It also describes penalties for failure to do so. Reporting is available to site administrators.
